Record Request
Requests for Client Records
All current and former clients of Marist180 have the right to access their records. Clients may request their records by contacting their designated Caseworker, Program or House they engaged with. If these details are not known the request can be submitted below.

Once you have contacted us we will call or email you on the contact details you have provided, to verify your identification.
Support while accessing your records
Accessing your records can bring up emotions, these feelings can be positive or can sometimes be upsetting or overwhelming. We recommend that you have a support person with you throughout the process. If you would like somebody from Marist180 to support you while you review these records, please let us know.
